The purpose of this page:
In accordance with Texas House Bill 4224 and Texas Health & Safety Code § 181.105, Rooted Love Counseling provides the following information regarding access to health care records, consumer complaints, and the Texas Behavioral Health Executive Council (BHEC).
Requesting records:
Clients have the right to request access to their health care records in accordance with applicable state and federal law.

To request copies of your records:

Submit a written request by email.

Include your full name, date of birth, and a description of the records you are requesting.

Identity verification may be required to protect client privacy and confidentiality.

Applicable fees may apply as permitted by law.

Records will be provided within the timeframe required by Texas law.
Requests submitted on behalf of another individual must include documentation demonstrating legal authority, such as guardianship documentation or a medical power of attorney.

Filing a consumer complaint:
Consumers who wish to file a complaint regarding services provided in Texas may do so through the Texas Office of the Attorney General.

The complaint process is independent of Rooted Love Counseling.
